Sciatica in Orange County: when to see a PT, when to wait it out.

Sciatica is the catch-all term for shooting pain that runs from the lower back down through the buttock and into the leg. It is one of the most-Googled musculoskeletal complaints in Orange County. It is also one of the most over-Googled, partly because the word covers a lot of different problems.

What sciatica usually is

Most of the time, sciatica is irritation of the sciatic nerve as it leaves the lumbar spine. The cause is usually a disc that is bulging or herniated, or muscle tightness around the nerve as it passes through the buttock (piriformis territory). It often shows up after a long drive, a long flight, a weekend of yard work, or a few months of sitting at a desk that was supposed to be temporary.

When to see a PT

A few signals worth taking seriously:

  • Pain shooting all the way to the foot, especially with numbness or tingling
  • Pain that wakes you up at night or makes sitting impossible
  • Pain that is no better after 7 to 10 days of normal activity
  • Pain that gets worse with bending, sitting, or sneezing

A PT can usually figure out within one visit whether you are dealing with disc irritation, piriformis tightness, joint involvement, or something else entirely. They can also tell you whether you need imaging (you usually do not need an MRI for the first episode of sciatica) or a physician referral.

"I'll just stretch it out" is the single most common way sciatica becomes a six-month problem.

Red flags — call a doctor, not a PT

A small set of symptoms means call your physician or go to urgent care, not a PT first: loss of bladder or bowel control, sudden severe weakness in the leg, numbness in the saddle region (between the legs), or pain that follows a recent fall or trauma. These can be signs of cauda equina syndrome, which is a medical emergency.
